Patient Leadership Council

With your consent, Crinetics will collect and use the information you submit here to indicate your interest in participating in the Patient Leadership Council (PLC), and to determine if you are eligible to serve on the PLC. If you are selected, you will be contacted by the Crinetics Patient Advocacy Team. Please make sure you review the PLC Privacy Notice and consent statement presented to you below before you submit your personal information. Crinetics will collect, record and use your personal data solely for the purpose of determining whether you may qualify to serve on the PLC as well as to prioritize diversity on each council. The use of your personal data for this limited purpose requires your explicit consent. Crinetics will record and use your personal data for the limited purpose as described above. This information may include: Your identity (such as your name, email address, phone number), your demographic data (such as your age, gender, race, or ethnicity) and your provided health data.

Only the Crinetics Patient Advocacy Team will have access to your personal data collected and will use it for the limited purpose described above. Your personal data will be protected and will not be shared with anyone outside the Crinetics Patient Advocacy Team.

Information about you will only be kept as long as necessary to conduct the limited purpose described above. At the end of this period, your personal data, including any health data, may be destroyed or anonymized.

You have the right to ask what data are used and why. You can request a copy of your personal data and/or ask for your information to be transmitted to another person of your choice. You can correct or complete your information. You can ask to limit further use of your information. You also have the right to revoke your consent to the processing of your information.
